Infection in Transplant
Pretransplant Infection Screening:
- Standard donor and recipient pretransplantation testing includes:
- Serologies for CMV, EBV, varicella-zoster, HIV, Hepatitis B/C/E, syphilis, toxoplasmosis
- IGRA for latent TB
- Strongyloides, Leishmania, and Trypanosoma if from an endemic area
Hematopoietic Stem Cell Transplant Infection:
- Risk after HSCT is much greater for allogeneic than autologous transplantation because of:
- Myeloablative conditioning regimen which results in a prolonged period of intense neutropenia
- Immunosuppression to reduce GVHD, resulting in a prolonged period of impaired cell-mediated and humoral immunity
