Presentation :
Pathophysiology :
- Primary Membranous Nephropathy: Approximately 75 percent of cases in adults. Result from humoral autoimmune response to a normal podocyte antigen in the absence of secondary features or etiologies of disease.
- Antigens implicated in primary MN include the phospholipase A2 receptor (PLA2R), thrombospondin type-1 domain-containing 7A (THSD7A), neural epidermal growth factor-like 1 (NELL1), semaphorin 3B (Sema3B), the serine protease HTRA1, protocadherin 7 (PCDH7), and others
- Secondary Membranous Nephropathy: Results from other etiologies: autoimmune diseases (eg: SLE), certain drugs, infections (such as hepatitis B), malignancy
Diagnostic Testing:
- Anti-Phospholipase A2R Ab titers are elevated in primary membranous nephropathy
