Polycystic Ovary Syndrome (PCOS)
Presentation :
- Hyperandrogenism (hirsutism, acne)
- Amenorrhea, oligomenorrhea, infertility
- Associated with metabolic syndrome (DM2, HLD, obesity, OSA)
Pathophysiology :
Diagnostic Testing:
- Patients should be screened for diabetes, dyslipidemia, obesity, hypertension, and OSA.
- in women with rapidly progressive hirsutism or virilization measure serum testosterone and DHEAS
- An androgen-secreting ovarian or adrenal tumor should be suspected in a woman with acute onset of rapidly progressive hirsutism or virilization.
Treatment :
- Weight loss is a first-line intervention.
- Metformin for prediabetes/diabetes
- If fertility is not desired, choose oral contraceptive for treatment of hirsutism and regulation of menses
- Add spironolactone if hirsutism remains a problem
- If fertility is desired, ovulation can be induced with clomiphene citrate or letrozole
