Tertiary Hyperparathyroidism
Presentation :
- End-stage kidney disease, poorly controlled hypocalcemia and hyperphosphatemia progressing to eucalcemia or Hypercalcemia
- May be unmasked by kidney transplantation
Pathophysiology :
- Chronic exposure of parathyroid to low calcium and high phos results in parathyroid hyperplasia. In some patients this then leads to the autonomous production of PTH by hyperplastic glands which characterizes this condition
Diagnostic Testing:
- High PTH despite eucalcemia or hypercalcemia in correct clinical setting
Treatment :
| - Cinacalcet treatment usually resolves the hypercalcemia |
